Job Title: Registered Nurse (RN) – Outpatient Heart Failure & Heart Transplant

Location: White Plains, NY (On-Site)

Job Type: Full-Time

Department: Cardiology – Heart Failure and Heart Transplant Services

Reports To: Nurse Manager / Clinical Coordinator

Position Overview:

We are seeking a compassionate, experienced, and detail-oriented Registered Nurse (RN) to join our outpatient Heart Failure and Heart Transplant team in New York City. In this vital role, you will provide specialized nursing care to patients with advanced heart failure, including those undergoing evaluation for or recovering from heart transplant. You’ll be part of a multidisciplinary team focused on optimizing patient outcomes, delivering education, and supporting long-term disease management.

Key Responsibilities:

  • Provide direct nursing care to heart failure and post-heart transplant patients in an outpatient setting.
  • Perform clinical assessments, obtain vital signs, review labs, and monitor signs and symptoms of decompensation or rejection.
  • Administer medications and therapies per protocol, including IV infusions and diagnostic prep as needed.
  • Educate patients and caregivers on medications, lifestyle modifications, symptom monitoring, and transplant-specific care.
  • Coordinate care across disciplines including cardiologists, transplant coordinators, social workers, and dietitians.
  • Assist with heart transplant evaluation by collecting and reviewing necessary documentation and test results.
  • Monitor and follow up on diagnostic testing, including echocardiograms, labs, and imaging.
  • Document all care, communication, and patient education accurately in the electronic health record (EHR).
  • Participate in team meetings, patient care conferences, and quality improvement initiatives.
  • Triage patient concerns and clinical issues via phone and in-clinic visits.

Qualifications:

  • Current RN license in New York State (required)
  • Bachelor of Science in Nursing (BSN) preferred
  • Minimum of 2 years of clinical nursing experience, preferably in cardiology, transplant, or critical care
  • Prior experience with heart failure or transplant populations is highly desirable
  • BLS and ACLS certification (required)
  • Strong clinical judgment, communication, and organizational skills
  • Proficient in use of electronic medical records (EMR) and Microsoft Office applications

Preferred Skills:

  • Experience working in a multidisciplinary outpatient setting
  • Familiarity with heart transplant protocols and patient education
  • Ability to manage complex clinical situations independently and escalate appropriately
  • Compassionate, patient-centered approach with cultural sensitivity
